A panel of the HCPTS heard that Mrs Carpenter’s failings included not undertaking or recording supervision in the case of a child at high risk of sexual exploitation. She also failed to ensure that an assessment of the risk posed to an unborn child by the father, a registered sex offender, was undertaken.

The Panel further heard that Mrs Carpenter recorded that a supervision meeting for Child A had taken place when this was not the case, and failed to maintain accurate records.

Panel Chair Ian Spafford commented:

“Mrs Carpenter’s shortcomings were numerous, repeated and occurred over a significant period of time. They were also serious and exposed vulnerable children to unnecessary risk of harm.”

The Panel ordered for the name of Lynn Carpenter to be struck off the HCPC Register.

Mrs Carpenter was neither present nor represented at the hearing.
